If you do not want to use the form, but would rather receive a PayPal invoice, then send an email to craigp@belleviewsoccerclub.com with the following information:
- Name
- Phone
- Style (color scheme)
- Size (be sure to include if this is an adult or youth size)
- Name on Back (optional)
- Number on Back (optional)
You will then receive a PayPal invoice the same day which may be paid either with a PayPal account or a major credit/debit card. All invoices MUST be paid by Friday 2/26 or that order will not be included in the first shipment.
If you need to pay in cash or with a check, please do so at the concession stand on any game night. Since the first order to the uniform company will be on Friday 2/26, the in-person orders (cash or check) will need to be done at the game tonight to be ordered for the first shipment.
